Utilidad del bloqueo diagnóstico y terapéutico de las facetas lumbares en el síndrome facetario

Introducción: Más del 70 % de las personas a lo largo de su vida padecen dolor lumbar. Resulta uno de los motivos más comunes de consulta médica y la causa más frecuente de limitación de la actividad en adultos laboralmente activos.
Objetivo: Exponer los resultados del bloqueo facetario en el diagnóstico definitivo como tratamiento alternativo y específico del síndrome facetario lumbar crónico.
Métodos: Estudio descriptivo, retrospectivo, que incluyó 103 pacientes con un cuadro clínico concordante con dolor de origen facetario, quienes acudieron a consulta de dolor en el Centro Internacional de Restauración Neurológica, a los que se le realizó un bloqueo de la articulación facetaria con anestésico local, esteroide y ozono.
Resultados: El 56,2 % de los tratados resultaron mayores de 50 años y 52,9 % eran mujeres. El 50 % requirió bloqueo facetario y discal y, de estos, el 17,6 % tuvo mala respuesta al tratamiento; sin embargo, el 51,9 % tuvo una excelente respuesta. La mejoría del dolor y la funcionalidad a los tres meses del tratamiento fue significativa. Se presentó un 6,86 % de pacientes con reacciones vagales y 3,92 % con hematoma en el sitio de la punción.
Conclusiones: En el dolor lumbar coexiste el origen facetario y el lumbar con más frecuencia. El bloqueo de la faceta articular con anestésico local, corticoides y ozono permitieron la remisión del dolor en más del 50 % sin recidiva, en un período de tres meses.
