Trasplante de progenitores hematopoyéticos autólogo en pacientes con linfomas indolentes de células B en Cuba

RESUMEN
Introducción: Los linfomas indolentes de células B son un grupo heterogéneo de neoplasias hematológicas. En determinados escenarios clínicos, el trasplante de progenitores hematopoyéticos autólogo constituye una estrategia terapéutica de consolidación.
Objetivo: Evaluar los resultados obtenidos en pacientes con linfomas indolentes de células B en los que se realizó el trasplante de progenitores hematopoyéticos autólogo en el Hospital Clínico Quirúrgico “Hermanos Ameijeiras”.
Métodos: Se realizó un estudio retrospectivo, entre 1985 y 2024, en 10 pacientes con linfomas indolentes de células B. Se analizaron la respuesta terapéutica a los tres meses postrasplante, la mortalidad a los 100 días, las causas de fallecimiento y la sobrevida libre de progresión a los cinco años.
Resultados: La cohorte presentó una edad promedio de 40,6 años. La principal indicación para el trasplante de progenitores hematopoyéticos autólogo fue la recaída temprana. Al momento del procedimiento, el 80 % estaba en remisión completa. El régimen acondicionante más utilizado incluyó ciclofosfamida y etopósido en altas dosis, complementado con irradiación corporal total (60 %). La fuente de progenitores más empleada resultó la médula ósea, estimulada o no, en igual proporción. La recuperación hematológica ocurrió en promedio a los 13,8 días para los neutrófilos y 16,6 días para las plaquetas. A los tres meses postrasplante, la mayoría alcanzó remisión completa. Las principales causas de muerte fueron una recaída/progresión y las malignidades secundarias. No se reportó mortalidad relacionada con el procedimiento en los primeros 100 días. La sobrevida libre de progresión estimada a cinco años fue del 70 %.
Conclusiones: El trasplante de progenitores hematopoyéticos autólogo demostró ser un procedimiento seguro y eficaz en pacientes con los linfomas indolentes de células B, al confirmar su utilidad como estrategia de consolidación en el contexto cubano.
