Hernia hiatal mixta complicada con vólvulo gástrico doble

Resumen

Introducción: El vólvulo gástrico es una complicación infrecuente de la hernia hiatal que puede comprometer la vida. Está definido como una rotación anormal de una parte o todo el estómago alrededor de uno de sus ejes.

Objetivo: Describir el caso de una paciente con diagnóstico de hernia hiatal tipo III o mixta con un vólvulo gástrico doble o combinado con incarceración del estómago.

Presentación de caso: Mujer de 70 años, ingresada por dolor torácico e intolerancia a los alimentos. El estudio radiológico contrastado demostró vólvulo gástrico doble como complicación de hernia hiatal mixta. Fue intervenida de urgencia, se le realizó la liberación y la reducción del estómago. Se cerró el defecto herniario y se realizó la funduplicatura. 

Conclusiones: El diagnóstico precoz del vólvulo gástrico en pacientes estables con evolución reciente de obstrucción gástrica permite el abordaje laparoscópico de forma segura.
