Abstract

Objective: Demonstrate the importance of the use of a conventional microscopy and immunohistochemical techniques for the diagnosis and classification of melanomas in the service of Anatomical Pathology.

Presentation of case: 77 year-old female that approximately 2 years ago reffered the growth of a lesion on the upper part of the back which had a nodular characteristic, heterogeneous coloration with pain to the touch. Macroscopically the sample sent had a size of 5x8x2cm, with a hardened central tumoral lesion with a light brown color with dark brown areas, stony 1.5cm x 1cm with edges free of lesion.

Results: The microscopic study showed desmoplastic melanoma, so immunohistochemistry of the sample was performed. The markers vimentin, melan-A, ki-67 and HMB-45 were studied; it was positive for vimentin, with positive fusocellular component for melan-A, as well as ki-67 with positive fusocellular component and negative HMB-45, presenting positive superficial epithelioid component.

Conclusions: The morphological characteristics by conventional microscopy and immunohistochemistry allow to identify desmoplastic melanoma, currently recognized as an entity with distinctive clinical, pathogenic and molecular characteristics, which has an impact on the effective treatment to the patient.
