Influence of Comorbidities on the Evolution of Patients with Rheumatoid Arthritis

Abstract

Introduction: Rheumatoid arthritis is a chronic inflammatory disease of unknown etiology and characterized by pain, inflammation and joint damage, accompanied by extraarticular manifestations and impaired quality of life.

Objectives: To evaluate the influence of comorbidities on the evolution of patients with rheumatoid arthritis and to estimate the clinical parameters of such disease activity.

Methods: An observational and prospective cohort study was conducted on a sample of 399 patients with a diagnosis of rheumatoid arthritis –according to 2010 criterions by the American College of Rheumatology and the European League against Rheumatism 2010– protocolized in the rheumatology service, in the period from November 2013 to February 2015.

Results: The most frequent age group was between 45 and 65. The white skin color and the female sex predominated. 66 % of the sample presented some associated comorbidity, being arterial hypertension the most frequent. The patients with longer evolution time had less improvement. The number of comorbidities and the initial clinical disease activity index were consistent predictors, with a significant statistical response. An increase in the frequency of cancer was found. Patients with comorbidities achieved less remission.

Conclusions: The associated diseases were arterial hypertension, peptic ulcer disease, hypercholesterolemia and diabetes mellitus.
