Abstract

Introduction: Exosome-based regenerative therapy has become popular as a therapeutic option with increased tolerance and safety due to the advantages of cell exclusion. It is a good therapeutic option to accelerate healing, improve skin quality, regenerate damaged tissues and treat musculoskeletal injuries. Even when obtained from adipose tissue mesenchymal stem cells, the use of nanovesicles seems to offer superior outcomes.

Objective: To present the current perspectives of regenerative therapy based on adipose tissue–derived stem cell exosomes, as well as their role in intercellular communication and their contribution to the regenerative potential of adipose tissue.

Methods: A literature review of articles in English and Spanish was carried out in PubMed, Elsevier, Google Scholar, Research and SciELO.

Results: Of the 187 found sources, 54 were selected according to the inclusion criteria, from 1987 to 2024. Adipose tissue surprises for its regenerative potential. The results, initially attributed to the presence of stem cells, seem to correspond, in addition, to the intercellular communication signals provided by the exosomes derived from them, enhanced by bioactive cell therapy.

Conclusions: The scientific evidence shows chronological advances that reaffirm adipose tissue as the most abundant, accessible, safe and available source of regenerative elements.
