Association of Neutrophil/Lymphocyte and Platelet/Lymphocyte Ratios with Disease Activity in Patients with Rheumatoid Arthritis

Abstract

ABSTRACT

Introduction: Neutrophils, lymphocytes, and platelets play an important role in the inflammatory process of rheumatoid arthritis.

Objective: To evaluate the association between the neutrophil/lymphocyte ratio and the platelet/lymphocyte ratio with rheumatoid arthritis disease activity.

Methods: A descriptive, cross-sectional study was conducted in patients with rheumatoid arthritis from the Rheumatology Service of Hermanos Ameijeiras Clinical-Surgical Hospital, from January 2021 to January 2023. Demographic and clinical variables, clinical inflammatory activity, and erythrocyte sedimentation rate were considered; hematological counts of neutrophils, platelets, and lymphocytes were determined; the neutrophil/lymphocyte and platelet/lymphocyte ratios were calculated; and their association with disease activity and clinical characteristics was studied.

Results: Seven hundred three patients were included. Neutrophil and platelet counts differed significantly between active and inactive patients (p < 0.021 and p < 0.023). The neutrophil/lymphocyte and platelet/lymphocyte ratios differed significantly between active and inactive patients: neutrophil-lymphocyte ratio 2.8 (± 1.5) vs. 2.6 (± 1.8) and platelet/lymphocyte ratio 180.7 (± 81.2) vs. 171.3 (± 80.5), p = 0.002, respectively. The neutrophil/lymphocyte ratio and platelet/lymphocyte ratio were associated with the presence of arthritis and morning stiffness (the neutrophil/lymphocyte ratio was 81 (40.9 %) p < 0.001 vs. 86 (39.4 %) p < 0.001, and the platelet/lymphocyte ratio was 22 (11.1 %) p < 0.003 vs. 24 (11%) p < 0.002).

Conclusions: The hematological markers analyzed were associated with the inflammatory activity of the disease.
