Abstract

Introduction: Acute febrile neutrophilie dermatosis, also known as acute febrile neutrophilic dermatosis, is a rare but significant condition due to its association with systemic diseases, including infectious, autoimmune, and hematological neoplastic disorders. Clinically, it is characterized by fever, skin involvement, and a neutrophilic dermal infiltrate on skin biopsy, making its identification crucial not only for its cutaneous manifestations but also as a potential indicator of an underlying severe disease.

Objective: To present a clinical case of a patient with Sweet's syndrome with clinical and histopathological criteria.

 

Case Presentation: A 39-year-old female patient presented with erythematous and infiltrated skin lesions on her face and arms, which progressed and extended to her back and lower limbs, accompanied by fever >38°C. A skin biopsy revealed a significant inflammatory infiltrate with a predominance of neutrophils. Treatment with dapsone was initiated (due to a personal history of allergy to steroid therapy, which limited the use of corticosteroids), followed by antihistamines and emollient creams, resulting in improvement of pruritus and skin lesions.

Conclusions: Acute febrile neutrophilic dermatosis is a rare dermatosis with a complex diagnosis given its varied clinical presentation, with multiple lesions that can be confused with other entities and that can lead to erroneous treatment, so it is necessary to take into account the clinical and histopathological diagnostic criteria.
