Overweight and Obesity in the Prognosis of Patients with Chronic Heart Failure and Atrial Fibrillation

ABSTRACT
Introduction: Chronic heart failure and atrial fibrillation share risk factors such as overweight and obesity, which are considered markers of poor prognosis. However, in secondary prevention, they act as protective factors against mortality.
Objective: To evaluate the association of overweight and obesity with the prognosis of patients with chronic heart failure and atrial fibrillation.
Methods: A prospective, observational, cohort study was conducted in 145 patients seen in the heart failure and/or arrhythmia clinics of the Hermanos Ameijeiras Hospital. The probability of being free of death was estimated using the Kaplan - Meier method. The relative risk was determined to evaluate the association between clinical and prognostic variables in relation to mortality and hospitalizations. The Cox regression model was used to evaluate the prognostic value of overweight and obesity.
Results: The probability of surviving one year of follow - up was higher in overweight (0.870) and obese (0.875) patients. Reduced ejection fraction and hospitalization on two or more occasions increased mortality in overweight and obese patients. In the Cox regression model, the overweight group showed an adjusted OR of 0.618, and the obese group showed and adjusted OR of 0.687, which was not significant in relation to mortality.
Conclusions: In the context of chronic heart failure and atrial fibrillation, overweight and obesity behave in secondary prevention as favorable prognostic markers in relation to mortality.
