Algorithm for Multimodal Treatment Strategy for Neuropathic Pain in People with Diabetes Mellitus

Abstract

ABSTRACTIntroduction: An algorithm is a finite and precise ordered set of instructions or steps followed to solve a problem, perform a calculation, or execute a specific task. Objective: To create an algorithm that organizes the multimodal treatment strategy for neuropathic pain in people with diabetes mellitus. Methods: A prospective longitudinal descriptive study was conducted with 160 patients at the Pain Clinic, “Hermanos Ameijeiras” Hospital, between January 2015 and December 2019. Results: Pain was located in the lumbar region (55.0 %), cervical region (20.6 %), and lower limbs (10.0 %). The most frequent diagnosis was compressive radiculopathies due to lumbar disc herniation (51.9 %), with a symptom duration of more than six months. The application of the neuropathic pain questionnaire at the different evaluation points with the proposed treatments demonstrated a significant decrease in symptoms (p < 0.001). Pain intensity, as measured by the visual analog scale, was also reduced (p < 0.001). The combination of antidepressants, anticonvulsants, and opioids, along with invasive treatments, proved useful for relieving neuropathic pain. The multimodal therapeutic strategies applied resulted in a reduction in pain intensity, frequency, and presence of neuropathic symptoms, leading to the proposal of treatment algorithms. Conclusions: The proposed algorithms constituted a valuable tool for the multimodal treatment strategy for neuropathic pain in people with diabetes mellitus, since they served as a guide for treating physicians who are not very familiar with this type of pain.
