Post-plication intra-abdominal pressure of the fascia of the rectus abdominis muscles in abdominal dermolipectomy

Abstract

Introduction: Reduction of the abdominal wall contour together with plication of the abdominal musculoaponeurotic system leads to an increase in intra-abdominal pressure and, to a variable degree, undocumented intra-abdominal hypertension.

Objective: To determine the variation of intra-abdominal pressure in patients who underwent abdominal dermolipectomy with plication of the fascia of the rectus abdominis muscles.

Methods: A prospective and longitudinal descriptive study was performed in the Plastic Surgery and Caumatology Service of the Clinical Surgical Hospital "Hermanos Ameijeiras", from March 2018 to March 2020. The sample was 56 patients, according to non-probabilistic selection criteria of the researcher.

Results: The mean preoperative intra-abdominal pressure was 4.8 (SD 1.4) mmHg, which had a statistically significant increase (p < 0.001) to an average of 10.5 mm Hg postoperatively. There was a correlation between plication size and intra-abdominal pressure. Intra-abdominal pressure increased on average by 0.653 mm Hg for each centimeter that the plication was increased. Intra-abdominal hypertension was higher with plication ≥10 cm. There was a range of reliability at a plication of less than 10 cm, since intra-abdominal hypertension was present in more than 50% of the patients.

Conclusions: We found that as the size of the rectus abdominis plication increased, the value of intra-abdominal pressure increased and, with it, the likelihood of intra-abdominal hypertension in the immediate postoperative period.
