Determination of interleukin-6 levels in burned patients

Abstract

Introduction: Burns activate the inflammatory response with defensive intentions; when exaggerated, it produces hemodynamic alterations. Interleukin-6 is a pro- and anti-inflammatory cytokine that increases from the acute phase, and is suggested as a prognostic indicator.

Objective: To determine Interleukin-6 levels in burned patients.

Methods: Longitudinal and prospective descriptive study. Patients of both sexes, older than 19 years and with burns of 10% or more of body surface area were included. Those who arrived after the first 24 hours, those with a history of inflammatory or immune disease and obese patients were excluded. Those who died before 72 hours of evolution were excluded. Interleukin-6 was measured during the first 24 hours, at 72 hours, and weekly for three weeks. The following variables were studied: age, extent of burn, prognosis, presence of major complication and state at discharge, with which the mean Interleukin-6 was related, applying the corresponding statistical tests according to the variable. The significance level was set at p = 0.05.

Results: The sample consisted of 19 patients. Interleukin-6 was highly elevated in all patients, with significantly higher values in the most severe patients and a tendency to decrease in the less compromised ones. The deceased patients showed significantly higher levels than the survivors; the same occurred in the complicated patients.

Conclusions: The burn patients studied evolved with very high levels of Interleukin-6, which demonstrates an association between the elevated values and the results, expressed in complications and death.
